Feedback on School List

I'm working on my school list for the upcoming cycle and would appreciate some feedback on mission fit/feasibility. I am most interested in primary care and ok with MD or DO--PS narrative will be about serving family/community.

Stats: 3.8x cGPA, 3.6x sGPA (idk the actual points because of classes I took outside undergrad) and 520 MCAT.

ECs:

- Recently became certified as an EMT and will have ~180 volunteer and ~400 paid hours by application time. May also gain some hours as a psych PCT.

- Human subjects research in two AD/Dementia labs. 1 poster, 1 publication 500+ hours. I have a couple other miscellaneous research hours I may add here and there.

- Service: Tutoring Math in underserved schools during undergrad + tutoring refugees in English now 150+ hours, AmeriCorps doing similar things at my public library 300 hours, service program on food insecurity which involved food bank volunteering 300 hours

-Leadership: idk how many hours an an officer for a prehealth community service club, 800+ hours as a lab supervisor doing research on mental health/minority rep in media

- a meager 40 hours of hospital volunteering (all my ECs are meager in this sub LOL)

- 84 shadowing hours

NOT URM, Red Flags: C+ in gen chem 1 freshman year but had a massive upward trend in chem grades, withdrew from physics senior year (oops) but am taking calc-based physics now at a cc.

I really do not want to take CASPAR or PREview, which is why VCU isn't on here (I also cut Drexel, Wake Forest, and MCW). I don't fully understand what "ties" you to a state for admissions purposes but I've moved around a lot. I lived in Iowa for a year and completed my AmeriCorps service there, had ties to WI as I was living on the border. I went to high school in PA. I also lived in Ohio and parents worked at UT/on the Michigan border. I also have extended family ties to MD and NY. Would love to know which schools I have the most realistic shot at and which I should cut or if I should just scrap it all and become an SLP instead.
